Worker's Compensation - Additional Information

Since the Rep Council Meeting of September 21, 2005 the Union has received additional information regarding the pre-designation of a physician to care for bargaining unit members in case of a work related health issue. The physician consent component of SB 899 is new and a change in the law. This means that any member who is insured by Pacific Care and wishes to have their personal physician be the designated care provider, that member must have his/her physician sign and date the SJUSD generated form.
SJTA has the forms that our members will need for this process [Click here to download form in .pdf format] . Please follow these simple instructions if you need to pre-designate a physician. 1) Secure the form from SJTA. 2) Call your physician and ask if he/she is agreeable to be the designated care provider should you have a workers comp health issue. 3) If he/she is agreeable, FAX the “completed form” to them for your physician’s signature and date. 4) The doctor’s office can then FAX the completed form to the District in care of Debbie Fleming, Risk Manager, at (916) 971-7013.
Kaiser members do not have to get a physician’s signature. They simply have to pre-designate their physician if they have not done so in the past. All members only have to do this once unless a change is necessary. Please call Sandra Galindo at 487-7582 if you have any questions. |
